Infectious Diseases Physicians, Inc. is a referral-based medical specialty practice.
Patients must have a formal referral from their healthcare clinician—such as a Physician, Nurse Practitioner, or Physician Assistant—typically from their Primary Care Physician (PCP) or another specialist. A referral ensures that specialty care is clinically appropriate and supports coordinated, team-based treatment.
This process:
- Confirms that an infectious disease consultation is necessary
- Promotes communication between providers
- Prevents fragmentation of care
- Ensures that evaluation and treatment align with our specialty expertise
To provide the most effective care from your first visit, we require the following:
- A referral from a healthcare clinician that includes:
- Patient demographics
- Insurance information
- Relevant medical records, including:
- Recent progress notes
- Laboratory results (including microbiology data)
- Radiology reports
- A clear reason for referral, outlining the specific question(s) or concern(s) to be addressed by the infectious disease specialist
- Insurance authorization, if required by your health plan for specialist visits
Once all required information is received, one of our infectious disease physicians will review the records to confirm that the referral is appropriate and within the scope of our specialty. Completing these steps in advance allows us to streamline the registration process and ensure a timely, efficient review—so we are fully prepared to provide high-quality care at your first appointment.
Please fax new referrals to fax number 703-560-8408 and include:
- Patient demographics and insurance(s)
- Progress notes
- Laboratory reports
- Radiology reports
- Insurance authorization if required from Primary Care Physician
